What is Outreach?

 

Outreach is defined as a method of seeking out older persons, identifying their service needs, and providing information and assistance to link those persons with services that address their needs.

 

Why Are Outreach Services Needed?

 

Every person should be able to access services appropriate to their needs so they can lead an independent, meaningful, and dignified life in their own home and community for as long as possible. Outreach is needed because often people are unable to seek out services for themselves, may be unable to access services, or may not be aware of the services available.

 

Outreach workers are willing to visit with you, provide information, assist with referrals and help connect seniors with services.  If you, a family member, friend, neighbor or someone you know needs a little bit of help to maintain their independence, please contact:
